The Barry County Health Department wishes to see if there is enough interest in the Monett community to host a COVID-19 immunization clinic for students ages 5-11.
The COVID-19 vaccine is NOT required and is entirely optional. By completing this survey, you are not committing to the vaccine but are showing interest if it were available to you.
If there is enough interest, the Health Department will set up a date and time and provide more information to the people who reply.
